These Terms & Conditions govern use of this website. By using the site, you agree to these terms. Separate written agreements, consents, and financial policies apply if you receive healthcare services.

1. Educational information only

Website content is for general educational and informational purposes. It is not medical advice, diagnosis, treatment, or a substitute for evaluation by a qualified healthcare professional. Do not delay or disregard professional care because of information on this website.

2. Emergencies

This website, email links, and discovery calls are not monitored for emergencies. If you believe you are experiencing a medical or mental health emergency, call 911 or go to the nearest emergency department.

3. No provider-patient relationship

Viewing the website, sending an inquiry, or participating in a free 15-minute discovery call does not create a provider-patient relationship. The discovery call is a general, no-obligation conversation to help determine whether Functional Medicine may be an appropriate option; it is not a clinical visit and does not include individualized medical advice.

4. Functional Medicine and Primary Care are separate

Patient Centered Health Functional Medicine is a virtual, self-pay service and is not billed to insurance. It does not replace Primary Care, urgent care, emergency care, or specialist care. Patient Centered Health Primary Care is a separate in-person service with its own scheduling, clinical, billing, and insurance processes.

5. Eligibility and location

Services are offered only when clinically appropriate and legally permitted in the location where the patient is physically present. We may decline or discontinue services, recommend in-person care, or refer you to another provider when your needs fall outside the scope or limitations of virtual Functional Medicine care.

6. Fees, scheduling, and cancellation

The discovery call is free and approximately 15 minutes. Fees for clinical services, laboratory testing, supplements, scheduling, cancellations, refunds, and payment obligations are governed by the written financial and service terms presented before purchase or enrollment.

7. No guarantees

Health outcomes vary. Testimonials, examples, educational content, and descriptions of potential benefits do not guarantee any specific result. Recommendations depend on individual circumstances and may need to change over time.

8. Supplements and laboratory information

Information about supplements, nutrition, wellness products, or laboratory testing is educational unless provided to an established patient as part of an individualized plan. Supplements are not intended to diagnose, treat, cure, or prevent disease, and may interact with medications or medical conditions. Discuss changes with an appropriate provider.

9. Acceptable website use

You agree not to:

  • Use the website for unlawful, fraudulent, abusive, or harmful activity.
  • Attempt to disrupt, probe, bypass, or gain unauthorized access to the website or related systems.
  • Copy, republish, sell, or commercially exploit website content without written permission.
  • Misrepresent your identity or submit information on another person’s behalf without authority.

10. Intellectual property

The website’s text, branding, logos, graphics, layout, and original materials are owned by or licensed to Patient Centered Health and are protected by applicable intellectual-property laws. Limited personal, noncommercial use is permitted; no other rights are granted.

13. Limitation of liability

To the fullest extent permitted by applicable law, Patient Centered Health and its owners, clinicians, employees, and service providers will not be liable for indirect, incidental, special, consequential, or punitive damages arising solely from use of or inability to use this public website. Nothing in these terms limits rights or liability that cannot legally be limited.

14. Changes and severability

We may update these terms when the website, services, or legal requirements change. Continued use after an update means you accept the revised terms. If any provision is unenforceable, the remaining provisions remain in effect to the extent allowed by law.
